Cold Water and the Breath: What Wim Hof Got Right, and What Can Kill You
Start Here, Before Anything Else
This article covers two practices that are almost always sold together: cold-water exposure and intense breathwork, popularized as a package by the Dutch athlete Wim Hof.
They should not be practiced together. Ever.
The breathing technique lowers the carbon dioxide in your blood — and CO₂ is the alarm that tells you to breathe. Suppress it, get in water, and you can lose consciousness with no warning and drown. This has killed people. We will explain exactly why, and we will keep the two protocols at the bottom of this page completely separate for exactly this reason.

The Appeal Is Real. So Is the Overselling.
Cold plunges and breathwork have become a wellness signature — the ice bath photo, the “I do Wim Hof every morning” testimonial, the promise of transformed immunity, mood, metabolism, and resilience.
Some of that is supported. A lot of it isn’t. And the honest version is more useful than either the hype or the backlash.
Let’s take the two practices one at a time, because their evidence — and their risks — are genuinely different.
Cold Water: Good for Recovery, Thin for Everything Else
The strongest, most repeated finding for cold-water immersion is athletic recovery. After hard exercise, cold immersion reliably reduces delayed-onset muscle soreness and the biomarkers of muscle damage over the following 24-48 hours. 1 If you have trained hard and want to feel less wrecked tomorrow, this is a real effect with real evidence behind it.
Beyond recovery, the picture gets murkier. The most comprehensive review to date — 3,177 healthy adults across 11 studies — found genuinely mixed results. 2 Cold immersion was associated with a reduction in stress about twelve hours later, and showed promising (but not yet solid) signals for sleep quality and overall wellbeing. But it found no consistent benefit for mood or immunity, and — importantly — a short-term increase in inflammation immediately after immersion, not a decrease. 2 The authors’ own conclusion is the honest headline: there is currently limited high-quality evidence for cold-water immersion in the general population, and several of its popular claimed benefits remain unsubstantiated.
Two more details worth knowing, because they cut against the marketing:
The inflammation spike is the point — and also a caution. The immediate rise in inflammation is the body’s stress response to the cold, similar to how exercise damages muscle before making it stronger. 2 That hormetic “small stress makes you more resilient” mechanism is plausible and interesting. But it also means cold isn’t universally “anti-inflammatory” the way it’s often sold.
Cold right after strength training can blunt your gains. Immersion immediately after resistance exercise has been shown to inhibit explosive power and doesn’t improve strength recovery in step with soreness relief. 3 If your goal that day was building strength or power, an ice bath right after may work against the adaptation you just earned. Save the cold for endurance recovery days, or put hours between the lift and the plunge.
So where does cold land? A legitimate tool for exercise recovery and possibly for stress and sleep, oversold as a cure-all, with a real trade-off if you use it at the wrong moment. That’s a Moderate evidence rating with an asterisk — not a miracle, not nothing.
The Breathing: This Is Where People Have Died
Now the part that requires your full attention.
The Wim Hof breathing method is, mechanically, controlled hyperventilation: rounds of 30 or so strong breaths, followed by a breath hold. 4 It does appear to do real things — controlled studies have shown it can influence the autonomic nervous system and reduce the inflammatory response to an injected bacterial toxin in healthy young men. 5 That is a genuine, interesting finding. It is not the same as being proven to treat any disease, and it does not make the technique safe in all contexts.
Here is the danger, stated plainly.
Hyperventilation strips carbon dioxide out of your blood. CO₂ — not low oxygen — is what triggers the urge to breathe. When you blow off your CO₂ and then hold your breath, you feel calm and can hold far longer than normal, because the alarm has been switched off. But your oxygen keeps falling the whole time. It can fall to the level where you lose consciousness before your body ever signals distress. 6
On dry land, seated or lying down, a blackout means you slump and start breathing again. In or near water, a blackout means you drown — silently, with no struggle, no warning. This is called shallow-water blackout, and it kills strong swimmers in water they could stand up in.
This is not hypothetical. One investigation documented thirteen drowning deaths of known Wim Hof practitioners who likely lost consciousness in water, plus additional deaths by cardiac arrest during ice exposure. 6 There is an active wrongful-death lawsuit alleging the breathing method caused a 17-year-old to drown in her own family pool. 7 Water-safety and free-diving experts — communities that have understood this risk for decades — describe hyperventilating before submersion as, in one director’s words, a recipe for blackout. 6
The rule that follows is absolute, and every credible source states it the same way: never do this breathing technique in water, before entering water, or anywhere you could fall into water. Dry land only. Seated or lying down. Somewhere a blackout would be harmless. 8
This is exactly why the two protocols below are separated. The single most dangerous thing in this entire practice is the instinct to do the breathing and then get in the cold. Do not build that habit. Do not let the two live in the same session.
The Safer Doorway: Slow Breathing
If the appeal of breathwork is calm, focus, and down-regulation, there is a whole family of practices that deliver much of that with a far cleaner safety profile — and they are, mechanically, the opposite of the Wim Hof method.
Slow breathing — extended exhales, box breathing, simple nasal breathing at around six breaths per minute — does not strip out your CO₂. It gently engages the parasympathetic “rest and recover” branch of your nervous system. 9 It is the down-regulation practice we point to in Train the Mind Like a Muscle and Meditation After the Hype. It won’t give you the dramatic head-rush of hyperventilation. It also won’t ever cause a blackout.
For most people, most of the time, this is the breathwork worth building into a daily life. The intense stuff is a specialized tool with a specific, serious risk — not a morning ritual to do half-asleep.
When a Product May Help
Neither practice requires anything to buy. Cold showers are free. Slow breathing is free. But two items lower the friction if you’re committed.

For structured cold exposure without plumbing a plunge tub, the simplest start is your own shower — no product needed. Protocol 1 — Cold Exposure (Water Only, No Breath Games)
The one rule for this protocol: breathe normally the entire time. No breath-holds. No hyperventilation. No Wim Hof rounds before or during. If you feel the urge to “prepare” with intense breathing, that is exactly the instinct that has killed people. Just get in and breathe.
Start with the shower, not the plunge.
- Week 1: End your normal warm shower with 15-30 seconds of cold — as cold as your tap runs. Breathe slowly and steadily. Step out.
- Week 2: Extend to 45-60 seconds of cold at the end. Keep breathing normal and controlled. The goal is calm, not gasping.
- Week 3+: Work toward 1-2 minutes if you like. More is not better — there is no evidence that longer or colder produces proportionally more benefit, and the risks rise.
If you progress to a plunge or cold bath: never alone, never after alcohol, and never immediately after a heavy strength session (it can blunt the adaptation). Water at or below 15°C counts as cold exposure; you do not need ice.
Timing note: for exercise recovery, cold works best after endurance sessions or hours after strength work — not immediately after lifting.
The reflection: after two weeks, ask — Do I feel more resilient to the cold, and calmer through it? The trainable skill here is staying composed under a sharp stressor. That composure is arguably the most transferable benefit, and you can feel it build.
Protocol 2 — Slow Breathing (Dry Land Only, Always)
The one rule for this protocol: never in water, never before entering water, never anywhere you could fall into water. Seated or lying down, on dry land, every time. This protocol uses gentle slow breathing, not hyperventilation — but the rule stays absolute so the habit is never built.
We are deliberately teaching the safer form of breathwork here — the down-regulating kind, not the Wim Hof hyperventilation method. If you choose to explore intense breathwork later, do it with proper instruction, never near water, and understand the blackout risk described above.
- Sit or lie down somewhere comfortable and safe. Feet on the floor or back on the ground.
- Breathe in through your nose for a count of four.
- Breathe out slowly through your nose or mouth for a count of six. The long exhale is what engages the calming response.
- Continue for five minutes. Roughly six breaths per minute. When your mind wanders, return to the count.
- Notice the settling. Not a head-rush — a gradual down-shift. That is the point.
The reflection: after two weeks of daily practice, ask — Can I reach for this when I’m stressed, and does it actually bring me down a notch? A breathing practice you can deploy in a tense moment is worth more than an impressive one you do once and abandon.
One more time, because it is the thing that matters most: the intense, hyperventilation-style breathing that Wim Hof is famous for is not this practice, and must never be combined with water. Keep your breathing practice and your cold practice in separate sessions, separate places, separate parts of your day.
Cold water and the breath are genuine tools. Used honestly, they build a kind of composure under stress that transfers to the rest of life. Used carelessly — the breathing near water especially — one of them can kill you.
Take the calm. Skip the recklessness. Keep the two apart.
The Lookout
The evidence here splits cleanly. Cold-water immersion for exercise recovery is well-supported — soreness and muscle-damage markers reliably improve. Cold for general health (mood, immunity, metabolism, longevity) is thin, mixed, and oversold; the best current review calls the general-population evidence limited and several popular claims unsubstantiated. The stress and sleep signals are promising but rest on small, short studies.
The Wim Hof breathing research is a strange case: there is a genuine, replicated finding (autonomic modulation and a dampened inflammatory response to an injected toxin in young healthy men), wrapped around a technique with a documented body count when misused near water. Both things are true at once. The finding is real; the danger is real; they are not in tension. A practice can be physiologically interesting and still kill you if you do it in the wrong place.
What we would flag for anyone drawn to this: the benefits people chase (calm, resilience, better sleep, feeling alive) are largely available from the lower-risk versions — cold showers rather than solo ice plunges, slow breathing rather than hyperventilation. The dramatic versions add risk faster than they add benefit. And the single most important piece of information on this entire page is not a benefit at all. It is this: never do the breathing in water.
